Botox isn’t just for wrinkles. One of the most requested treatments I do has nothing to do with lines on your face—it’s for the jaw muscle itself. Masseter Botox can soften a square or wide jawline and calm down clenching and grinding at the same time.
If you’ve been Googling “jaw Botox” at 1am because your jaw is sore from grinding, or because you’ve wanted a way to slim your lower face, here’s what you need to know.
What Is Masseter Botox for Jaw Slimming and TMJ?
The masseter is the thick chewing muscle at the back corner of your jaw. You can feel it bulge if you clench your teeth right now. When that muscle overdevelops from years of clenching or just genetics, it can make the jawline look wider or squarer than you’d like.
Botox injections relaxe some of those muscle fibers so they contract less forcefully. Over time, the muscle gradually shrinks, and the jawline softens.
Two Different Reasons People Get It
- Jawline slimming: Helps to soften a wide or square lower face that’s resulted from overactive masseter muscles. It gives the lower face a more narrowed appearance to help achieve a more heart-shaped facial shape and more balanced proportion.
- Clenching, grinding, and jaw tension: Calming an overactive masseter can ease the muscle soreness and tension that comes with clenching or grinding your teeth, including some TMJ-related discomfort.
A lot of patients come in for one reason and end up just as happy about the other—the two effects go hand in hand.
What to Expect
Dosing
Most patients start in the 15-25 unit range per side, depending on how strong and developed the muscle is and what you’re hoping to achieve. I always start conservative, especially on a first treatment, then dial up the from there.
*Note that some patients experiencing pain, tightness, or headaches may also need the muscles in their temples treated to fully alleviate their symptoms. This is always evaluated prior to treatment and the dose for this area usually starts at 10-20 units per side.
Timeline
You’ll usually feel less tension within 1–2 weeks. The visible jawline slimming takes longer to show up. Most patients see the biggest change around the 8–12 week mark, once the muscle has had time to shrink down.

Results typically last 3–6 months before a touch-up is needed, similar to Botox anywhere else on the face.
For either treatment purpose, I recommend that my patients stick to a 3-4mo treatment schedule for the first 2-3 treatments to gain good control over the muscle size/activity, then treatments can often be spaced out to every 6mo.
Is It Right for You?
Good candidates usually have:
- A wider or squarer jawline that comes from muscle bulk, not bone structure
- Noticeable clenching, grinding, or jaw soreness—especially first thing in the morning
- Realistic expectations: this slims muscle, it doesn’t reshape your bone or tighten skin
If your jaw width is mostly bone structure rather than muscle, masseter Botox won’t make much of a visible difference. That’s something we can talk through honestly at your consultation before you book anything.

Does masseter Botox hurt?
It feels similar to Botox anywhere else. Just a few quick pinches and you’re done. Some patients say the jaw muscle itself feels a little more sensitive than the forehead, but it’s still quick and very manageable.
Will it change my smile or how I chew?
With careful mapping, no. You may notice mild chewing fatigue for the first couple weeks while the muscle adjusts, especially with tougher foods, but normal function comes back quickly.
How long does it last?
Most patients maintain results with a treatment every 3–6 months. Some find that after a few consistent rounds, the muscle stays smaller for longer between sessions. It can ease muscle-based clenching and tension that contributes to jaw soreness. It doesn’t treat issues inside the joint itself. So if you have diagnosed TMJ disorder, this often works best alongside your dentist or TMJ specialist.
What about mouth guards for TMJ?
Mouth guards can help with mild cases by reducing wear on your teeth and absorbing some of the impact from grinding. The catch is that the added cushion gives the masseter something to push against. This can actually cause the muscle to work harder and hypertrophy even more. That’s why some people end up biting straight through their guard. It protects your teeth, but it doesn’t address what’s driving the problem.
What are the potential risks of Masseter Botox for Jaw Slimming (and TMJ Relief)?
The most common side effect is temporary weakness in the surrounding muscles, which can occasionally affect chewing or cause mild fatigue in the jaw with tough foods. This usually resolves on its own as the treatment settles.
There’s also a small risk of the injector hitting the wrong muscle, which can cause asymmetry in the lower face. This is why placement matters and why you want someone with real anatomy knowledge doing this, not just anyone with a syringe.
Bruising and soreness at the injection site can happen, though it’s typically minor and short-lived.
