This is one of those topics where there’s very little good information online, and a lot of unnecessary stigma around even asking about it. So let’s just talk about it plainly.
What Is Anal Botox?
Anal Botox involves injecting Botox (or Dysport) into the internal anal sphincter—the muscle ring that controls the anal opening. Like every Botox application, it works by temporarily relaxing the muscle. It’s been used in clinical and surgical settings for decades, primarily for conditions like chronic anal fissures and an overly tight sphincter.
Clinical research shows botulinum toxin produces meaningful healing in a large share of chronic fissure cases, and with repeat treatment, healing rates climb even higher (you can read the research here).
In recent years, it’s also become an increasingly popular elective treatment for men, related to comfort and function during sexual activity.
Who Gets Anal Botox and Why?
Functional / Clinical
- Chronic anal fissures: overactive sphincter tension can prevent fissures from healing. Botox relaxes the muscle and lets the tissue repair.
- Chronic tightness or spasm: persistent sphincter tension can cause ongoing discomfort. Relaxing it provides significant relief.
Elective / Comfort-Based
- Men who experience discomfort during receptive anal sex due to involuntary sphincter contraction
- Men who want more comfort and ease without ongoing tension
- Patients who’ve tried other approaches and want something more controlled and lasting
Both categories are valid reasons to pursue treatment. There’s no judgment about which one applies to you.
What Does the Treatment Involve?
The procedure is done in a clinical setting with topical numbing applied beforehand. Injections use a very fine needle (the size of an insulin needle) to inject Botox or Dysport into the internal sphincter muscle. The whole treatment takes about 15 minutes and the injections themselves take less than 30 seconds. Some mild discomfort or pinching during the procedure is normal. For most, the general response is “Oh that wasn’t as bad as I thought it would be!”
How Long Does It Last?
Results typically last 3–4 months, consistent with other Botox applications. After that, muscle activity gradually returns to normal, and many patients return regularly to maintain the effect.
Is It Safe? Are There Side Effects to Anal Botox?
Yes, it is safe when it’s performed by a trained provider who understands the anatomy well. Botox has an extensive safety record, and this application is well-studied in clinical literature.
Regarding side effects, there are very few. As with any injectable procedure, there is a risk of bruising bleeding, or infection. A more uncommon, but possible side effect is more frequent flatulence.
Many patients’ #1 question is “Does it cause incontinence?”. While it’s not impossible, this would be an incredibly rare side effect, with most studies citing it’s occurence at <1% and some studies even citing 0%. Remember, it’s a strong, thick muscle that is working all day, every day without us thinking about it. We’re just reducing tension and making it easier to relax the muscle, not completely taking away it’s function.
